Who is responsible for your information
This website is run by Dr J S Deogan Ltd, the company through which Dr Jagdeep S Deogan (GDC no. 243908) provides his services. The company is the data controller for information you send through this website and is registered with the Information Commissioner’s Office (registration number ZB712419). You can contact me about privacy at drjsdaesthetics@gmail.com.
If you go on to become a patient, your clinical records are held by the practice where you are treated, Bridge Dental & Implant Clinic or Parklands Dental Care, under that practice’s own privacy notice.
What I collect
- Consultation enquiries: your name, phone number, email address, preferred practice and contact time, and anything you choose to tell me about your concerns, dental history or how you feel about dental visits.
- Referrals from dentists: the referring dentist’s details, and the patient’s name, contact details and clinical information needed to assess the referral.
Information about your teeth and gums is health information, which the law treats as special category data. I ask for only what is needed to respond to you.
How and why I use it
- To reply to your enquiry and arrange a consultation (lawful basis: your consent, and steps taken at your request before providing care).
- To assess and act on referrals (lawful basis: legitimate interests and, for health information, the provision of health care).
- Health information you volunteer in an enquiry is used with your explicit consent, which you give by ticking the consent box on the form.
I never sell your information or use it for marketing without your separate agreement.
Who I share it with
Your enquiry goes to me and to the team at the practice you choose, so they can contact you. The website and its forms are hosted by Netlify, Inc., which processes the information on my behalf. Netlify may store data outside the UK; where it does, it is protected by the UK International Data Transfer Addendum or an equivalent safeguard. I do not share your information with anyone else unless the law requires it.
How long I keep it
Enquiries that do not lead to treatment are deleted within six months. If you become a patient, your information becomes part of your clinical record and is kept in line with NHS and professional record-keeping guidance.
Your rights
You can ask to see the information I hold about you, have it corrected or deleted, restrict or object to its use, or withdraw your consent at any time. To do so, contact me using the details above. If you are unhappy with how your information has been handled, you can complain to the Information Commissioner’s Office at ico.org.uk or on 0303 123 1113.
Cookies
This website does not use analytics, advertising or tracking cookies. Its fonts and images are served from this site, not from third parties.
The practice maps on the home page are provided by Google Maps. They load only if you accept cookies, or choose Show map. When they load, Google may set cookies and receive information such as your IP address, under Google’s privacy policy.
